Description:

St. Francis Hospital in Wilmington, Delaware, a member of Trinity Health Mid-Atlantic is seeking a Post-Acute Network Liaison to join our Accountable Care Organization!

  • Responsible for coordinating and implementing actions to develop and maintain Quality Health Alliance's preferred network of skilled nursing facilities (SNFs), long-term care hospital (LTACH), and inpatient rehabilitation facility (IRF).

  • In collaboration with the interdisciplinary care teams manages and coordinates integration of all levels of care to assure smooth transitions.

  • Promotes a collaborative, open, and inclusive work environment within a highly matrixed organization by maintaining strong relationships with post-acute leaders, medical directors, and all members of the clinical team.

  • Works collaboratively with acute care and ambulatory care management teams to improve communication across the continuum of care.

Minimum Qualifications

  • Must possess a comprehensive knowledge of value-based care arrangements with experience in healthcare including post-acute care, hospital care, primary care, and health care plans as normally obtained through a Bachelors or Master’s degree in nursing or Social Work or Physical Therapy or a related field , and 5 or more years of post-acute utilization management and/or integrated care coordination experience.

  • Two (2) years of program management level experience within a major health care organization, health system setting, or large multi-operational corporate environment in complex industries like healthcare.

  • Must be able to travel to the various Trinity Health sites (50%) as needed.

Required Skills:

  • Program management level experience

  • Excellent communication and interpersonal skills as demonstrated in oral, written and electronic communication.

  • Competency in utilization management and transition of care management

  • Ability to work in electronic platforms, EMRs, and Microsoft 365 (Excel, Word, PowerPoint, Outlook, Teams)

  • Ability to manipulate and analyze data in Excel, as well as present data to leaders using PowerPoint

  • Knowledge of post-acute care reimbursement structures, best practice transition of care models and state, county and community resources preferred
