What you will do: This position requires interactions with frail, elderly or disabled people in their homes. The individual must possess strong interpersonal skills in order to meet the physical and psychological needs of the participant and family. We provide personal care and/or homemaker services. We are looking for a dependable ,gentle, respectful individual whom loves the elderly. Come join our team !

Responsibilities:

The Certified Home Health Aide is a paraprofessional health care worker prepared to function in a supportive relationship to professional nursing and other health care disciplines.

  1. Participates in the activities as directed by the physical therapist, occupational therapist and nurse including but not limited to: range of motion exercises strengthening and endurance exercises, general mobility, transfer and ambulation to and from the bus as appropriate both in home and/or in the day center.

  2. Assist with maintenance level ambulation and transferring programs in the home and in the day center.

  3. Assists with rehabilitation activities and exercise groups by setting up, assisting the instructor and cleaning up.

  4. Assist with application of Jobst stockings, TED stockings, or in the use of other assistive devices as necessary.

  5. Maintains necessary documentation and record keeping related to patient care and treatments. Maintain documentation with regulatory requirements.

  6. Maintains/Assists with daily and routine cleaning and tracking of rehabilitation equipment i.e. hydroculator, paraffin equipment, mats, chairs, parallel bars, walkers and wheelchairs, etc.

What you will need:

  • High School diploma or GED preferred

  • Completion of Home Health Aide Training Program, Home Health Aide Equivalency Program, C N A Transition Program, or Personal Care Aide Upgrade Program approved by the New York State Department of Health

  • New York State Drivers License

  • One or more years experience is preferred, as either a Home Health Aide in a Certified or Licensed Home Health Agency or as a Certified Nursing Assistant in a hospital or nursing home.

Eddy Senior Care is located at 1938 Curry Rd in Rotterdam. Eddy Senior Care is the Capital Region's only Program of All-Inclusive Care for the Elderly (PACE). PACE is a special model of care proven effective at helping older adults with long-term health care needs live safely at home.

Pay Range:$18.55 - $21.95

Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
