Team 2 encompasses Berwyn, Broadview, Riverside, but also Chicago.

Position Purpose:

The Palliative Care RN is responsible for patients and families in the palliative care program. Provides Palliative care clinical consultation.

What You Will Do:

  • Collaborates with manager in the development of performance measurements for the Palliative Care Program to ensure continuous quality improvement; ensures implementation

  • Motivates/supports employees for exemplary performance on internal and external customer service, productivity, and accuracy

  • Provides statistical/budgetary data to appropriate Palliative care program

  • Works collaboratively with all departments to ensure execution of quality delivery of services

Minimum Qualifications:

  • Graduate of an approved nursing education program.

  • Current Registered Nurse licensure in the state of practice.

  • CPR certification

  • Hospice/Palliative Care certification preferred.

  • Home care experience, strong clinical knowledge base, Hospice experience, computer skills, strong interpersonal and organizational skills,

  • Palliative Care experience, demonstrate ability to use problem solving, critical thinking and priority setting skills

  • Must be able to work independently and on a team

  • Ability to perform diversified duties with time limitations

  • Ability to develop, coordinate and evaluate programs in Palliative care and end-of-life care
