Description:
RN, Admission Coordinator
Remote Admission Support Nurse (RN)who works offsite to help support the nurses in the facility with the admission process. The Remote Admission Support Nurse will assess and improve admissions at the facilities.
Essential Functions
Knows, understands, incorporates, and demonstrates the Mission, Vision, Values, and Guiding Behaviors of Trinity Health and Trinity Senior Living Communities in behaviors, practices, and decisions.
Consistently demonstrates (leads by example) Trinity Values to all internal and external customers (residents, visitors, volunteers, vendors, and associates). Creates and maintains an atmosphere of warmth, propagating a calm environment throughout the community.
Maintains a working knowledge of Trinity Health Organizational Integrity Programs, Standards of conduct, as well as other policies and procedures in order to ensure adherence in a manner that reflects honest, ethical, and professional behavior.
Respond to inquiries in a timely and professional manner through various methods of communication. Ensure that potential customers receive the highest level of customer service.
Demonstrate commitment to the quality improvement process and the philosophy of continuous improvement; identifies and responds actively and with sensitivity to the needs of all concerned; participates as a team player in all phases of the organization; and is open and responsive to change.
Must function independently, make independent decisions, and demonstrate flexibility, personal integrity, and ability to work effectively with residents and personnel.
​Primary Functions:
Follows a Standardized Admission Process established by Remote Admissions Program. Will support nursing manager to complete a full admission process.
Be a clinical resource for admission process and assessments. Supports multiple facilities in the admission process.
The admission tasks will be separated between an offsite nurse and a facility nurse.
Admission Nurse will access EPIC/hospital EMR when possible An initial patient chart review may include Diagnosis,Orders,Hospital EMR- H&P of resident stay, labs, xrays
Admissions Nurse will enter their assigned tasks into the Electronic Medical Record.
Minimum Qualifications
Must be a Registered Nurse with at least 5 years experience, Bachelors degree preferred or equivalent experience in Nursing. Willing to be licensed in state the THSC operates in.
Prefer recent nursing experience with the elderly in Skilled Nursing Facility or Rehabilitation.
