Position Purpose
This position supports the ambulatory Anticoagulation Service and Anticoagulation Transitions of Care Service by providing medication management under a collaborative practice agreement.
What you will do
Work collaboratively with ambulatory and inpatient consultant hematology team to provide safe initial prescribing and ongoing medication management of warfarin, direct oral anticoagulants, and low molecular weight heparin
Work collaboratively with hematology, cardiology and hospitalist teams to ensure safe transitions of care from hospital to home
Apply critical thinking to develop safe and effective therapy plans in real-time to implement during patient appointments
Order and monitor labs and prescriptions as outlined in the Collaborative Drug Therapy Management (CDTM) Agreement
Precept multiple learners including IPPE, APPE, PGY1 and PGY2 pharmacy residents
Minimum Qualifications
B.S. degree in Pharmacy or Doctor of Pharmacy Degree
Job Knowledge/Experience:PGY2 Ambulatory Care Residency training or equivalent experience preferred
Current Connecticut Pharmacist License
Board Certification preferred, would be required to obtain CACP credentialing within one year of eligibility to participate in collaborative practice agreement
