Position Purpose

This position supports the ambulatory Anticoagulation Service and Anticoagulation Transitions of Care Service by providing medication management under a collaborative practice agreement.

What you will do

  • Work collaboratively with ambulatory and inpatient consultant hematology team to provide safe initial prescribing and ongoing medication management of warfarin, direct oral anticoagulants, and low molecular weight heparin

  • Work collaboratively with hematology, cardiology and hospitalist teams to ensure safe transitions of care from hospital to home

  • Apply critical thinking to develop safe and effective therapy plans in real-time to implement during patient appointments

  • Order and monitor labs and prescriptions as outlined in the Collaborative Drug Therapy Management (CDTM) Agreement

  • Precept multiple learners including IPPE, APPE, PGY1 and PGY2 pharmacy residents

Minimum Qualifications

  • B.S. degree in Pharmacy or Doctor of Pharmacy Degree

  • Job Knowledge/Experience:PGY2 Ambulatory Care Residency training or equivalent experience preferred

  • Current Connecticut Pharmacist License

  • Board Certification preferred, would be required to obtain CACP credentialing within one year of eligibility to participate in collaborative practice agreement
