Description:

Position Purpose: Empower individuals and communities to enhance health knowledge and self-sufficiency through outreach, education, counseling, and advocacy. Facilitate communication, link clients to vital resources, advocate for local health needs, and provide support to chronically ill patients, including home visits.

What You Will Do:

  • Conduct outreach, community education, and informal counseling.

  • Advocate for local health needs and empower clients.

  • Link clients to healthcare and social service resources.

  • Provide patient care and support activities tailored to various age groups.

  • Collaborate with the Project LAUNCH Grant team to deliver care in diverse settings.

Minimum Qualifications:

  • Professional and cultural competence.

  • Ability to work independently and within a team.

  • Participation in organizational goal planning and implementation.

  • Understanding of collaboration importance and partnership development.

  • Bilingual (Spanish)proficiency in written and verbal communication.
