Registered Nurse-Liaison-Hospice-Intake- Samaritan and Ellis Hospitals

The Community Hospice has a rewarding opportunity for a RN to work as a part of our Hospice Intake RN Team serving as Liaisons in local hospitals. Hospitals this position will serve are both Samaritan Hospital in Troy and Ellis Hospital in Schenectady

What you will do:

  • Assessments, care coordination, and collaboration

  • Evaluate hospital referrals for hospice services

  • Describe the support and care available from hospice along with the benefits of hospice care.

  • Admission of patients to services in the hospital if applicable

  • Coordinate with the patient’s community physician and our hospice medical directors to confirm eligibility for hospice services.

  • Gather information and ensure a smooth handoff to the hospice RN admitting the patient/family to services.

  • Work closely with the hospital case management department.

  • The work schedule is M-F either 8a-4:30p or 9a-5:30p

What you will need:

  • A current license to practice as a Registered Nurse in the State of New York

  • Associates Degree in Nursing, BSN preferred

  • 1 year experience as an RN , preferably in Hospice/Home Care or acute care.

  • The RN must be able to communicate effectively, verbally and written

  • Valid Driver's License and Reliable vehicle

  • High level of interpersonal skills to establish and maintain relationships with patients, families, physicians, and coworkers

  • Understanding of Hospice principles preferred

Training provided in end of life care.

Pay Range:$33.10 - $45.75 Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
