Description:

The Registered Professional Nurse Case Manager (RN CM) assumes the responsibility and accountability for the care coordination (case management) of the Terrace assisted living residents. The RN CM provides leadership and guidance to the care staff team, as well as serves as a resource to the staff and provides ongoing resident information to staff, residents, and families. The RN CM has the primary responsibility for the resident Individualized Service Plans (ISP / care plans) and ensures the care staff work plans are aligned with the ISP's. The RN CM has the primary responsibility of communicating with resident physicians to ensure physician orders match the ISP and MAR, and communicating with families when changes occur.

Education Requirements:

  • Associates' Degree in Nursing (RN)

  • Current/Unencumbered NY state license as an RN

Experience Requirements:

  • Successful performance in nursing for 2 years

  • Experience with the geriatric population required

  • Work in the area of Case management

  • Assisted Living experience preferred

Principal Responsibilities:

  • Coordination of resident care (case management):

  • Looking for a nurse with a passion for seniors

  • Assisted Living environment geared towards building long term relationships with residents

  • Resident advocate and liaison

  • The RN CM assists the Terrace Director overseeing the daily operation of the Terrace and assumes responsibility when the Director is off site.

  • Provides leadership and guidance to the care staff team, as well as serves as a resource to the staff and provides ongoing resident information to staff, residents, and families.

  • The RN CM will, in coordination with the Terrace Director, establish the care staff work plans to ensure they are aligned with ISP's.

  • The RN CM has the primary responsibility of communicating with resident physicians to ensure physician orders match the ISP and MAR, and communicating with families when changes occur.

  • Assists with assuring compliance with DOH regulations.

  • Manage and coordinates admission of new residents by performing initial assessment and determining eligibility in compliance with admission and retention standards

  • Review new admission paperwork.

  • Attend management huddles and work with the team to coordinate new resident move ins.

  • Assemble new resident charts

  • Create initial Individualized Service plans, and keep ISPs up to date every 6 months minimum as well as annually and as needs of the resident changes.

  • Schedule physician appointments and transportation, or delegate to appropriate staff to schedule these appointments. Including annual appointments to keep DSS (med evaluation) current.

  • Assure that case management notes are current , complete and accurate.

  • Address family and resident concerns as they arise and escalate to Director as needed.

  • Be aware of changes in resident needs and keep resident, family and physician advised as needed

  • Schedule and facilitate weekly interdisciplinary case management meetings.

  • Medication administration

Pay Range:$34.05 - $46.75

Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
