Description:

This position will be located at the Trinity Health Breast Center - Lakes Village located in Muskegon, MI.

This is a part time position working a schedule of 3 days/week.

The Nurse Navigator is a key member of the multidisciplinary team functioning as an advocate, educator and facilitator of care. Responsible for ensuring all referred patients receive quality and comprehensive services throughout the continuum. Coordinates the overall subspecialty programmatic goals and outcome. This role works in a fast-paced environment within a growing organization.

POSITION QUALIFICATIONS

  1. Bachelor’s degree in Nursing – strongly preferred .

  2. Current State of Michigan Registered Nursing License; Certified Nurse in subspecialty of nurse navigation preferred.

  3. Proficient in breast exams and certification in clinical breast exams within 1 year.

  4. Nurse Navigation certificate within 1 year.

  5. A minimum of three (3) years' experience in an acute care setting . Case management experience or patient care leadership experience preferred. Five years' experience in subspecialty is preferred.

  6. Ability to navigate a standard computer desktop.

  7. Basic understanding of email, e-learning, intranet and computer navigations.

  8. Ability to use other software as required to perform the essential functions of the job.

  9. Must be able to work flexible schedules and at multiple campuses.

  2. Ability to comfortably and effectively interact with diverse populations.

  3. Demonstrated ability to do critical thinking and ability to assist patients through a continuum of care for the assigned caseload.

  4. Must demonstrate superior communication and facilitation skills.

  5. Must be able to prioritize and manage caseload with multiple interruptions.

  6. Must exhibit advanced communication and teaching skills; both verbal and non-verbal.

  7. Demonstrated expertise in caring for identified population of patients.

  8. Demonstrated ability to impact practice through change management and influence.

  9. Ability to reach, stand, walk, lift and carry over 20 pounds, but not over 35 pounds, and bend at the waist.

  10. Frequent computer work.

  11. Can effectively maintain the hours and workload expected of a busy healthcare team member.

  12. Manual dexterity and fine motor skills required.

  13. Ability to hear and distinguish speech and sounds, speak loudly and softly.

  14. Must be able to work in a fast paced environment with multiple interruptions.

  15. Potential exposure to blood and body fluids, radiation and toxic materials.

  16. Subject to multiple interruptions, changes in workload, competing priorities and deadline situations.

  17. Exposed to a variety of attitudes, personalities and reactions from patients, providers and employees.

  18. Manages multiple tasks at the same time while showing respect and courtesy to each

individual.
